Innovat Corporation always welcomes new business, whether an individual welder owner or a large corporation.  We require two forms to set up a new account, (1) a credit application and (2) a Federal W-9 Form for U.S. Customers or a Federal W-8 BEN Form for Foreign Customers. 

Individuals who are not setting up an open account and not reselling repaired p.c. boards / units do not have to fill out a W-9 form. 

Businesses may provide your own credit application and we have provided a link to download the W-9 / W-8 forms if you do not have one readily available.

We can fax, mail, or e-mail you the new customer forms and W-9 / W-8 at your request if you call our customer service department at 1-800-626-1887 or click on the links below to obtain.

Please note that the W-9 Form must include a Federal ID # or Social Security # and must include an authorized signature.  The W-8 BEN Form must include a foreign tax identifiying # and include an authorized signature.

If you are located in the state of Virginia and are state tax exempt, please fax a copy of your tax exempt form.
